ADOP Prepare Phase Fails with FATAL ERROR

I need your support regarding an issue encountered while executing the ADOP (Online Patching) prepare phase in Oracle E-Business Suite R12.2. The process failed with a FATAL ERROR, and the relevant details are as follows:

autoconfig + fs_clone may fix this..

No pending session exists.

We need to see the log..

There may be something like "Creating the new APPL_TOP Context file from" there in the log.. What happens after that, we need to see..

At the moment, this is generic as it is.
