Are you using Oracle Virtual Box on Hyper-V? Cause that document you sent is exactly for that..

anyways; more diag is needed..

running kfed on the problematic disks,checking ASM Alert log and that strace in the doc, may help us on this. -> strace -fo /tmp/a.out asmcmd afd_scan.
