If your version is 12.2; Did you check the following MOS note? ->

PO Approval Notification is not sending out email with completion of PO Output for Communication (Doc ID 2594175.1)

This might be related with the WF Mailer.. So what do you see in the mail status column of wf_notifications when you run a query by joining following tables -> po_headers_all poh, wf_notifications wn,po_lookup_codes pol ?
