Check your fnd_concurrent_queues as well..
the value of NODE_NAM may be wrong for OPP.. This can also be the cause.. Also check values of other columns and ensure all are set to the value they suppose to be set.

that note I ve sent you will help you in any case, cause there we have redirections to Concurrent Manager Recovery wizard, and that may also fix certain things in your concurrent processing environment.
