1. Apply Patch:5042134 on top of 10.1.2.0.2 .

   Alternatively you can apply the Patch 5488476 which supersedes Patch 5042134, and contains other important Forms fixes.

or

Apply Patch:5841985 on top of 10.1.2.2

For Forms 10.1.2.3, Patch:7121788 has to be applied.

2. After applying patches, the following steps are mandatory.

cd $ORACLE_HOME/bin 
chmod +x genshlib
