Thanks for your help and suggestions but the issue is resolved. I installed the EBS R12.2 using startCD 12.2.0_51 then I faced the issues mentioned above after successful application of AD and TXK packs. Then I proceeded with R121.2.4 upgrade pack and the forms are opening. I presume that the R12.2.4 upgrade pack contains the patch which can resolved the problem of not opening of forms after the application of AD and TXK packs.
