Thanks for the update.Now i have got some idea

1)How these block device names sdhm , sdcy , sdff  and sdar are assigned?
2)As said,They all point to the same device.so in our case they all pointing to 360002ac0000000000000007e0001c9fc.please correct me if we are wrong
