"Did you clear the apache cache, as well as your client's browser cache and restart the apache ?"

Do the things above, if the issue persists then do the following!

1. Run the Security List Maintenance concurrent program from the System Administrator responsibility
2 . While running the program be sure to choose the right parameters.Like 'Generate List for'
parameter must have a value which depends upon the kind of security profile you are
running. If your security profile is global you need to choose 'All Global
Security Profile' from the lov. For non-global choose All Security Profiles.
