adop error

I am applying a patch using downtime
adop phase=apply apply_mode=downtime patches=PATCH_NUMBER

Checking for pending adop sessions...
Reading driver file (up to 50000000 bytes).
    No pending session exists.
    Staging new adop session...
    [ERROR]     Failed to execute SQL statement :
       begin
         ad_zd_adop.INSERT_ADD_NODE_RECORDS;
        end;

    [ERROR]     Error Message :
    [UNEXPECTED]Error occurred while adding new node information
    [UNEXPECTED]Unrecoverable error occured. Exiting the current session.


[STATEMENT] Please run adopscanlog utility, using the command


I am following doc (Doc ID 2030341.1)

I can see I have only 1 node:
